As per this Subjunctive mood is necessary. (D) is the best.

Hi, Oksana,

In this case D should be the answer. Actually "on condition that" can be used with simple tenses, e.g. I'll come to the party on condition that you don't wear that ridiculous bow tie! Subjunctive mood is used here to stress the formal nature of the actor's request.
